These are all the downloadable forms you need to complete your MUSIC MANUFACTURING SERVICES Vinyl Manufacturing order. When you first contact MUSIC MANUFACTURING SERVICES, your rep will give you an Order Confirmation Number. This number should go on every form you complete.

Graphics Form
Your graphic designer should complete this form, then you include it with your artwork disc, or upload it to the MUSIC MANUFACTURING SERVICES design department using EASI along with the artwork.

Intellectual Property Rights Form (IPR)
This form establishes that you own or control the rights to reproduce the material contained in your order and is used to prevent unlawful copying. Your vinyl order will NOT BE PROCESSED without an IPR, and any required mechanical or master use licenses. (see Before You Order for more information).

Audio Anti-Piracy Form
This form supplies us with a track list for your vinyl recording, as well as copyright information on all of your tracks.
